COMMUNITY HEALTHCARE ACCESS PROGRAM

CHAP is a state run Healthcare Program that provides Healthcare at a reduced cost for Delaware residents.

Who is Eligible for CHAP?

The Community Healthcare Access Program (CHAP) is a program that provides healthcare to  Delaware residents that are between 100% to 200% poverty level.  CHAP is available to enrollees that are not eligible for Medicaid and are without healthcare benefits from another source.

    •    Those without other health insurance.

    •    Those with an annual income that meets program guidelines.

    •    Even if you are not a U.S. citizen, you may be eligible for CHAP.

What does CHAP Provide?

Some of the reasons we feel that the CHAP program is a great opportunity are because CHAP provides:

    •    Access to a doctor for checkups, physicals and sick visits.

    •    CHAP will help each enrollee to choose a medical home in their area that will be able to get to know
        their clients better and to take a more individual approach to their healthcare coverage.

    •    Each medical home can then refer any client with special needs out to the appropriate specialist.

    •    CHAP provides healthcare coverage at a reduced fee, based on your income and makes healthcare
        services affordable.

    •    Translated applications, brochures and support for their Spanish-speaking clients are also available. 

 

How Do You Get Enrolled Into CHAP?

     •       Call 1 - 800 - 996 - 9969.

     •       Choose a Medical Home in your Area 

     •     Make an appointment to see your CHAP Doctor.
